For your convenience, our central billing office is staffed Monday through Friday from 8 a.m. to 4:30 p.m. You can call the office at 419-557-5530.

You can download, print and fill out some of your patient registration forms prior to your appointment to reduce wait times. Downloadable patient forms include patient registration for adults, patient registration for pediatrics, privacy notice acknowledgement form, authorization for release of medical information, and the Firelands Physician Group financial policy.

Advance directives are legal written documents that indicate what an individual wants regarding care and treatment if he or she is not able to speak for himself or herself. There are three documents: a healthcare power of attorney, a living will and a do-not-resuscitate order. All must be signed by the competent patient and witnessed or notarized.
